You may complete the form online & pay online (additional 2.2% + $0.30 PayPal fee) or complete the form online and pay by check with remittance form.

You may also complete the form manually and pay by mail. 

If you have more than one disabled participant complete the online form for one participant and email us to add the other. You only need to submit one membership fee for the family.
